What you’ll do:

✔️ Conduct patient assessments & reassessments

✔️ Develop and implement care plans

✔️ Coordinate care with physicians & interdisciplinary teams

✔️ Educate patients and families

✔️ Ensure quality, safety & regulatory compliance

What we’re looking for:

✅ Active, unrestricted NY RN License

✅ 1+ year nursing experience preferred

✅ Home care experience strongly preferred

✅ Current CPR/BLS

✅ Reliable transportation
