Community Health Worker (CHW)

Location: Merced County

Schedule: Full-Time | Monday-Friday | 8:00 AM - 5:00 PM

Must be available to rotate occasional late evenings and weekends for community outreach.


Position Overview

We are seeking a compassionate, community-focused Community Health Worker (CHW) to join our care management team. In this role, you will build relationships with community partners, provide outreach to members, and support individuals by connecting them with healthcare, housing, and social resources. The ideal candidate has strong case management experience, excellent documentation skills, and a passion for serving underserved populations.


Responsibilities

  • Conduct community outreach and develop relationships with local partners, agencies, and organizations
  • Monitor daily call logs received from health plans and respond to member needs in a timely manner.
  • Manage a caseload of members, providing ongoing support and care coordination.
  • Complete and process Release of Information (ROI) forms and individualized care plans for authorization submissions.
  • Assist members with housing navigation, including connecting them with housing resources and support services.
  • Coordinate services with healthcare providers, community organizations, and social service agencies.
  • Maintain accurate, detailed, and timely documentation within the electronic care management system.
  • Advocate for members by helping them overcome barriers to accessing healthcare and community resources.
  • Participate in outreach events, community engagement activities, and team meetings as needed.


Qualifications

  • Bachelor's degree or Community Health Worker (CHW) Certification preferred, but not required.
  • Minimum of 2 years of case management experience.
  • Previous experience working in community health, social services, care coordination, or related settings preferred.
  • Experience supporting underserved or vulnerable populations is highly desirable.
  • Individuals with lived experience are encouraged to apply.
  • Strong computer skills with the ability to navigate multiple systems and complete detailed documentation.
  • Excellent communication, organizational, and interpersonal skills.
  • Valid driver's license, reliable transportation, and current auto insurance required.


Preferred Qualifications

  • Experience working within CalAIM programs.
  • Previous Community Health Worker (CHW) experience.
  • Knowledge of community resources, housing services, and care coordination.


Schedule

  • Full-time, 40 hours per week
  • Monday-Friday, 8:00 AM-5:00 PM
  • Flexibility to work occasional evenings and weekends for outreach activities
