Join our San Francisco, CA team! Our client is seeking a compassionate, bilingual Social Worker Assistant to support older adults through care coordination, community resources, and interdisciplinary services.
Requirements to Apply
- Bachelor’s degree in social work or a related field
- Current BLS and First Aid certification
- Fluent in English and Cantonese and/or Mandarin
- Strong communication, organizational, and computer skills
Social Worker Assistant supports full-time PACE care coordination, community resources, transitions, and independence for older adult participants.
Job Overview
Assist participants with accessing community resources and social services while supporting social workers with case management and care coordination. Responsibilities include hospital discharges, transitions of care, home visits, phone support, family meetings, care planning, and accurate documentation in Epic and other electronic systems. Collaborate with healthcare providers, community agencies, families, and the interdisciplinary care team while maintaining HIPAA and confidentiality standards.
