The Admissions Director is a senior, high-impact leadership position with end-to-end accountability for Medicare census development, referral conversion, and hospital network expansion. This is an active, revenue-critical role designed for an experienced skilled nursing professional who masters hospital discharge workflows, clinical gatekeeping, and payer mix strategy. The ideal candidate combines commercial drive and analytical discipline with clinical empathy and sharp judgment—winning referrals through responsiveness, relationship depth, and flawless follow-through.


Responsibilities

  • Drive Census & Revenue: Own Medicare admissions targets and lead daily census development to achieve optimal occupancy and revenue targets.
  • Convert Referrals Rapidly: Serve as the primary decision-maker for hospital referrals, prioritizing intake speed, clinical appropriateness, and high conversion rates.
  • Cultivate Health System Partnerships: Build, maintain, and expand active pipelines with hospital discharge planners, social workers, case managers, and physicians across Brooklyn and the greater NYC area.
  • Eliminate Intake Bottlenecks: Systematically identify barriers to admission—from clinical complexity to transport logistics—and resolve them proactively to eliminate referral leakage.
  • Manage Clinical & Financial Triage: Partner with nursing leadership, therapy teams, and finance to evaluate patient acuity, payer eligibility, and reimbursement viability prior to acceptance.
  • Analyze Performance Metrics: Track conversion ratios, referral trends, inquiry velocity, and payer mix analytics to identify growth opportunities and market shifts.
  • Ensure Operational Compliance: Maintain complete alignment with CMS guidelines, Medicare/Medicaid regulations, and facility intake protocols.
  • Lead Market Strategy: Collaborate with executive leadership on competitive positioning, programmatic service line development, and annual census forecasting.


Qualifications

  • Minimum 3–5 years of progressive admissions, intake, or hospital liaison leadership within a skilled nursing facility (SNF).
  • Proven track record of Medicare census development; deep fluency in Medicare A/Advantage guidelines, HMO authorizations, and commercial insurance is essential.
  • Established network of hospital case management and discharge relationships across Brooklyn and the NYC metro market strongly preferred.
  • Strong acumen in post-acute metrics: conversion rates, average length of stay (ALOS), case-mix index, and managed care contract nuances.
  • Decisive leadership style with the ability to thrive independently in an urgent, high-volume healthcare environment.
  • Exceptional communication, negotiation, and de-escalation skills when working with families, hospital teams, and clinical departments.


Compensation & Benefits

  • Base Salary: $90,000 – $125,000, commensurate with experience, existing hospital networks, and demonstrated track record of census growth.
