Home Health Nurse

We are seeking an experienced Registered Nurse (RN) – Home Health to provide high-quality, patient-centered care in the home setting.

📌 Position: RN – Home Health

📍 Location: New York

🩺 Experience: Minimum 2 years of RN experience

📋 Employment Type: Contract / Travel opportunities available

Pay: Upto $51/hr

🔹 Key Responsibilities

• Provide safe, competent, and evidence-based nursing care to patients in their homes

• Assess patient needs and develop, implement, and evaluate individualized plans of care

• Collaborate with patients, families, providers, and interdisciplinary care teams

• Provide nursing interventions including IV therapy, peripheral IV access, lab specimen collection, and wound care

• Monitor patient progress and ensure care is delivered according to provider orders and the plan of care

• Educate patients and caregivers on treatment plans, medications, disease prevention, and post-discharge care

• Manage an assigned caseload and ensure patients progress toward established goals

• Document patient care accurately in the EMR

• Identify and escalate safety concerns, barriers to care, and changes in patient condition

• Participate in quality improvement, evidence-based practice, and patient safety initiatives

🔹 Requirements

✅ Minimum 2 years of Registered Nurse experience

✅ Current New York State RN License

✅ BLS certification from AHA or Red Cross

✅ Relevant clinical experience and strong nursing assessment skills

✅ Excellent communication, interpersonal, critical-thinking, and problem-solving skills

✅ Ability to work independently while collaborating effectively with interdisciplinary teams

✅ Proficiency with Electronic Medical Records (EMR) and basic data-entry systems

✅ Ability to perform clinical procedures such as venipuncture and other nursing interventions

✅ Ability to travel to patient homes and meet the physical demands of the role
