Schedule: Monday–Friday | 8 am–5 pm

We are seeking a Level of Care RN for a full-time position offering $43/hr–$53/hr.

Requirements to Apply

• Bilingual Spanish/English proficiency required
• ASN, BSN, or MSN from a professional nursing program
• Active California RN license
• Current CPR (BLS) and First Aid training
• Valid California driver’s license and reliable vehicle
• Minimum 1 year RN experience preferred
• Community health, frail/elderly care, case management, hospice, home care, or home health experience preferred

Job Overview

The Level of Care RN will conduct in-home level of care assessments, coordinate with interdisciplinary teams, manage complex cases, ensure regulatory compliance, and support compassionate, patient-centered care for frail and elderly patients.
