A mission-driven community healthcare organization in New York City is seeking a Registered Nurse to support patient care coordination and care management initiatives.


The organization provides comprehensive healthcare services to diverse and underserved patient populations, with a strong focus on improving access to care and supporting individuals and families who may face barriers to receiving healthcare.


This position works closely with clinical providers, care teams, and support staff to coordinate patient services, identify higher-risk patients, and help ensure continuity of care.


Key Responsibilities

  • Review upcoming patient schedules and assist with pre-visit preparation and planning.
  • Monitor daily appointments and help coordinate patient care activities.
  • Prepare and review patient lists to identify individuals requiring additional clinical support or follow-up.
  • Participate in and document patient care discussions with members of the clinical team.
  • Conduct proactive outreach to patients with complex or higher-risk healthcare needs.
  • Coordinate care for designated patient populations requiring additional clinical management.
  • Partner with healthcare providers to develop individualized patient care plans.
  • Work directly with patients to establish treatment goals and identify potential barriers to care.
  • Educate patients regarding treatment recommendations, care plans, and available resources.
  • Maintain accurate and timely documentation of patient interactions and care-management activities within the organization's electronic health record system.
  • Participate in quality improvement and clinical performance initiatives.
  • Collaborate with patient support, benefits, outreach, and administrative teams to facilitate appropriate scheduling and continuity of care.
  • Coordinate with health information management staff to support accurate clinical documentation.
  • Perform additional responsibilities and special projects as needed.


Qualifications

  • Active and unrestricted New York State Registered Nurse (RN) license required.
  • Previous clinical nursing experience preferred.
  • Experience in care management, care coordination, community health, ambulatory care, or a similar environment is a plus.
  • Previous healthcare administrative experience is also beneficial.
  • Strong communication, organization, documentation, and patient-engagement skills.
  • Comfortable collaborating across clinical and non-clinical healthcare teams.
