Position: Case Manager (RN)

Location: Morristown, NJ 07960

Job Type: Contract

Contract: 13 Weeks

Schedule: Monday–Friday | 8:00 AM – 4:00 PM | Rotating Weekends & Holidays

Guaranteed Hours per Week: 37.5

Pay: Local: $60/hourly

Travel: $2400/weekly


Job Summary

Pride Health is seeking an experienced RN Case Manager for a 13-week contract opportunity at Morristown Medical Center in Morristown, NJ. The Case Manager will coordinate patients throughout their acute care hospitalization and manage all aspects of discharge planning, care transitions, utilization management, and quality screenings. The ideal candidate will have strong inpatient case management and acute-care discharge planning experience, including experience coordinating rehabilitation facility placement and caring for subacute patients.


Responsibilities

  • Coordinate patients throughout their acute care hospitalization.
  • Manage all aspects of the discharge planning and transition-of-care process.
  • Perform utilization management and quality screenings.
  • Assess patient needs and develop appropriate discharge and transition plans.
  • Coordinate placement in rehabilitation and other post-acute care facilities.
  • Collaborate with physicians, nurses, social workers, therapists, and other interdisciplinary team members.
  • Identify barriers to discharge and develop solutions to facilitate timely transitions.
  • Support appropriate utilization of healthcare resources.
  • Provide case management services for acute and subacute patients.
  • Maintain accurate and timely documentation in the medical record.
  • Participate in interdisciplinary rounds and case management activities.
  • Communicate effectively with patients, families, healthcare providers, and post-acute facilities.
  • Follow hospital policies, regulatory requirements, and established case management standards.


Requirements

  • BSN (Bachelor of Science in Nursing) required.
  • Active RN License required.
  • Minimum 2 years of acute inpatient hospital discharge planning experience required.
  • Inpatient Case Management experience required.
  • Hospital experience required.
  • Experience with discharge planning and placement in rehabilitation facilities required.
  • Experience with subacute patients required.
  • Experience with case management and/or managed care preferred.
  • Strong utilization management and care coordination skills.
  • Excellent communication, organization, and critical-thinking skills.


Preferred Qualifications

  • CCMC (Certified Case Manager) certification preferred.
  • Experience working in a hospital-based case management department.
  • Strong experience coordinating rehabilitation and post-acute placements.
  • Experience managing complex discharge planning situations.
  • Experience working with multidisciplinary healthcare teams.
