Contract: 13 Weeks with potential extension

Schedule: Monday–Friday | 9:00 AM – 5:00 PM

Guaranteed Hours per Week: 35

Pay: Local: $2,200 - Travel: $2,400 per week

Job Summary

Pride Health is seeking an experienced RN Care Manager (Inpatient) for a contract opportunity in Bronx, NY. The Care Manager will coordinate inpatient care, facilitate utilization review, discharge planning, and care coordination while collaborating with multidisciplinary teams to ensure high-quality, patient-centered care. The ideal candidate will have extensive clinical nursing experience and a strong background in case management within an acute care setting.

Responsibilities

• Coordinate and manage inpatient care throughout the patient's hospital stay.
• Perform utilization review to ensure appropriate level of care and resource utilization.
• Develop, implement, and monitor individualized patient care plans.
• Coordinate discharge planning and facilitate smooth transitions of care.
• Collaborate with physicians, nurses, social workers, therapists, and other interdisciplinary team members.
• Identify barriers to discharge and coordinate appropriate community resources and post-acute services.
• Maintain accurate and timely documentation in accordance with hospital and regulatory requirements.
• Participate in multidisciplinary rounds and case management meetings.
• Promote quality outcomes while ensuring compliance with hospital policies and payer guidelines.
• Communicate effectively with patients, families, and healthcare providers regarding treatment plans and discharge needs.

Requirements

• Active New York State Registered Nurse (RN) License required.
• Bachelor's Degree in Nursing from an accredited nursing program required.
• Minimum five (5) years of clinical experience as a Registered Nurse.
• Experience with Care Coordination, Utilization Review, and Discharge Planning required.
• PRI & Screen Certification required at the time of submission.
• Primary Source Verification through the New York State Department of Education (NYSED) required.
• Current BLS Certification required.
• Strong computer literacy and documentation skills.
• Excellent communication, critical thinking, and organizational skills.

Preferred Qualifications

• Bachelor's or Master's Degree in Nursing.
• Case Management Certification (CCM or equivalent).
• Leadership or supervisory experience.
• Previous acute care hospital case management experience.
• Experience working with interdisciplinary healthcare teams in an inpatient setting.
