Position: RN Care Manager (Inpatient)
Location: Bronx, NY 10451
Job Type: Contract
Contract: 13 Weeks with potential extension
Schedule: Monday–Friday | 9:00 AM – 5:00 PM
Guaranteed Hours per Week: 35
Pay: Local: $2,200 - Travel: $2,400 per week
Job Summary
Pride Health is seeking an experienced RN Care Manager (Inpatient) for a contract opportunity in Bronx, NY. The Care Manager will coordinate inpatient care, facilitate utilization review, discharge planning, and care coordination while collaborating with multidisciplinary teams to ensure high-quality, patient-centered care. The ideal candidate will have extensive clinical nursing experience and a strong background in case management within an acute care setting.
Responsibilities
- Coordinate and manage inpatient care throughout the patient's hospital stay.
- Perform utilization review to ensure appropriate level of care and resource utilization.
- Develop, implement, and monitor individualized patient care plans.
- Coordinate discharge planning and facilitate smooth transitions of care.
- Collaborate with physicians, nurses, social workers, therapists, and other interdisciplinary team members.
- Identify barriers to discharge and coordinate appropriate community resources and post-acute services.
- Maintain accurate and timely documentation in accordance with hospital and regulatory requirements.
- Participate in multidisciplinary rounds and case management meetings.
- Promote quality outcomes while ensuring compliance with hospital policies and payer guidelines.
- Communicate effectively with patients, families, and healthcare providers regarding treatment plans and discharge needs.
Requirements
- Active New York State Registered Nurse (RN) License required.
- Bachelor's Degree in Nursing from an accredited nursing program required.
- Minimum five (5) years of clinical experience as a Registered Nurse.
- Experience with Care Coordination, Utilization Review, and Discharge Planning required.
- PRI & Screen Certification required at the time of submission.
- Primary Source Verification through the New York State Department of Education (NYSED) required.
- Current BLS Certification required.
- Strong computer literacy and documentation skills.
- Excellent communication, critical thinking, and organizational skills.
Preferred Qualifications
- Bachelor's or Master's Degree in Nursing.
- Case Management Certification (CCM or equivalent).
- Leadership or supervisory experience.
- Previous acute care hospital case management experience.
- Experience working with interdisciplinary healthcare teams in an inpatient setting.
