Position: Case Worker

Location: New York, NY 10016

Job Type: Contract

Contract: 13 weeks with strong possibility of extension

Schedule: 7:00 hours/day | 5 days/week

Guaranteed hours per week: 35.00

Pay: Up to $38/hour


Job Summary

Pride Health is seeking an experienced Case Worker for a contract opportunity in New York, NY. The Case Worker will provide social services support to patients and families, assisting with care coordination, resource placement, and eligibility determination for various programs.


Responsibilities

• Provide social services to patients, including those requiring public assistance or institutional care

• Conduct assessments to determine patient needs and eligibility for services

• Develop and implement individualized service plans

• Assist with placement in nursing homes and long-term care facilities

• Coordinate referrals and case management activities

• Collaborate with healthcare teams to support patient care plans

• Provide counseling and support to patients and families

• Maintain accurate documentation and case records

• Utilize systems such as EPIC and Careport for referrals and coordination


Requirements

• Bachelor’s Degree from an accredited college or university

• Minimum 3 years of case work or social services experience

• Experience with nursing home placement and care coordination

• Strong communication and interpersonal skills

• Ability to manage multiple cases effectively


Preferred Qualifications

• Experience with EPIC and Careport systems

• Background in hospital or healthcare settings

• Strong knowledge of community resources and social service programs
